This screening tool is part of the Adult Pre-Exercise Screening System (APSS) that also includes guidelines (see User Guide) on how to use the information collected and to address the aims of each stage. To identify individuals with known disease, and/or signs or symptoms of disease, who may be at a higher risk of an adverse event due to exercise. An adverse event refers to an unexpected event that occurs as a consequence of an exercise session, resulting in ill health, physical harm or death to an individual.
This stage may be self-administered and self-evaluated by the client. Please complete the questions below and refer to the figures on this document. Should you have any questions about the screening form please contact your exercise professional for clarification.

Describe your current physical activity/exercise levels in a typical week by stating the frequency and duration at the different intensities. For intensity guidelines consult figure 2.
If your total is less than 150 minutes per week then light to moderate intensity exercise is recommended. Increase your volume and intensity slowly.
If your total is more than or equal to 150 minutes per week then continue with your current physical activity/exercise intensity levels.
It is advised that you discuss any progression (volume, intensity, duration, modality) with an exercise professional to optimise your results.
1. Access fob remains the property of Ballarat Body & Soul health and Fitness Studio at all times. 2. The access fob cannot be sold or transferred without the said approval of the above mentioned health club. 3. Replacement fobs will only be issued on full payment of a new fob. 4. All 24/7 members must be 18 years and over and where required will provide full photo identification. 5. Any member sharing a fob for a second party to gain access to the above club will have their membership cancelled with no refund of membership or fob. 6. Any person caught using a fob that is not theirs will be reported to police for entering a premise without permission and charged by the above club. 7. Any person causing any damage to the club will have their membership ceased with no refund of membership or fob. 8. Any person causing damage to the club will be reported to police and charged appropriately and incur all costs. 9. Any person interfering with security or duress systems unnecessarily will be charged for all cost’s incurred and have their membership cancelled with no refund and refund of fob. 10. Any person caught loitering in car park,surrounding area of the club or making a nuisance of themselves will be reported to police and have their membership cancelled with no refund and no refund to fob. 11. Any person caught trying to enter restricted areas of the club will have their membership cancelled with no refund and no refund to the fob. 12. Ballarat Body & Soul health and fitness studio reserves the right at all times to cease the membership of any person causing unnecessary harm or nuisance to others. The club will not refund the membership or fob. 13. Illegal substances and alcohol are not permitted on the premise or the outside premise known as Ballarat Body & Soul health and fitness studio, any person caught using or selling will have their membership ceased and reported to police. 14. Personal training is available during 24/7 all sessions must be paid for in advance and will not be transferable or refunded. 15. 24/7 VIP Card does not include personal training.
